New Patients: Click the scheduling button to be redirected to our secure scheduling system. Enter your contact information to create an account, verify with a one-time code, and complete your health history, case details, and insurance information before scheduling your initial evaluation.

Returning Patients: Log into the patient portal with your one-time code, confirm your contact information, and select your preferred physical therapist and clinic location to schedule your follow-up visit.
